You must have the latest version of the free Adobe Acrobat reader (version 5 or later) to read this form. When you select the Adobe Reader link, you go to a page to download Version 9.

If you would like us to fax an unofficial copy of your transcript, the faxed copy is $5.00.
If you attended OC prior to fall 1996 there will be a $2.00 fee per transcript.
